Current Vaccinations 

Provide copy of current vaccinations at GAI and leave in home during services

Emergency Contacts

Self, family, in town contact, regular and emergency Veterinary Hospital

Include 2 persons authorized to make medical/financial decisions for your pet in the event you are unable to be reached in an emergency

Veterinarian Notification

Call your regular Veterinarian and Emergency Hospital of choice to provide notification of your absence, and leave a credit card on file with THEM (not GAPC) for emergencies
